DOCX conversion to PDF with high fidelity resource

Attached docx file with variant column numbers table and checkboxes was converted to PDF by your evaluation resource http://converter-eval.plutext.com/plutext/converter of the PDF Converter.
As I see, rows 2.5 and 4 of the table were displaced, besides, checkboxes were not converted.
Can it be fixed somehow by source preparation or by changing PDF Converter?

The checkboxes in your docx use <w:instrText xml:space="preserve"> FORMCHECKBOX </w:instrText> fields.

We'll support this in a future version; in the meantime, you can use the Unicode checkbox characters instead.
